Good afternoon,Miramichi Rural Families.
We have learned that there will be vaccination clinics held next week on June 15th, 16th and 17th at the Miramichi Agricultural Exhibition Building, and we have been approved to send any students 12+ years who have yet to receive their first dose.
ASD-N will be busing students to and from the clinic for those wanting to get the vaccine. Staff supervision will be provided. Students who are 12-15 years of age will require consent from a parent or guardian.
Anyone receiving the vaccine must give consent using the form below. Students aged 12-15 must have their parent or guardian fill out and sign the consent form.

It is very important that signed consent forms are returned with the student no later than Monday, June 14th.
IMPORTANT- Please take your Medicare card with you.
